What the website collects
If you submit a coverage-audit request, Daybook Health, Inc. receives the name, work email, organization, facility-count range, optional message, referring page, and source label you provide. We use that information to respond to your request and evaluate whether an audit is a fit. The form service also records your IP address and browser user-agent for security, abuse prevention, and operational logging.
Analytics and cookies
If enabled, this site uses Google Analytics to understand how visitors find and use the pages, and HubSpot to manage business-contact records and site activity. These tools may set cookies in your browser. When enabled, we use them to see which pages are read, which sample documents are opened, and whether a request form was completed.
We do not run advertising on this site. When Google Analytics is enabled, it is configured to deny advertising storage, advertising identifiers, and ad personalization, and to shorten visitor IP addresses. We do not use these tools to build advertising audiences and we do not sell personal information.
If you submit the request form, the work email you provide is associated with your site activity in HubSpot so that we can respond in context. You can browse without submitting anything, and you can ask us to delete that record at any time using the address below. Most browsers also let you block or clear these cookies directly.
Clinical files and protected information
Do not place patient or resident information in a website form or ordinary email. Identifiable files are accepted only after an applicable Business Associate Agreement and engagement terms are in place and only through a separately designated, access-controlled channel.
Service providers and disclosure
Website infrastructure, analytics, and business-communication providers — including Amazon Web Services and, when enabled, Google Analytics and HubSpot — may process limited technical or business-contact data to operate their services. None of them receive patient or resident information from this website, because this website does not accept it. Daybook does not sell personal information. Information may be disclosed when required by law or to protect the security and integrity of the service.
Retention and requests
Business-contact information is retained only as long as reasonably needed for the relationship, legal obligations, and ordinary business records. To ask what information we hold, request correction or deletion, or raise a privacy question, email daniel@daybookhealth.com.
